For good and valuable consideration, which I have received, I hereby authorize Cutera and its subsidiaries and affiliates and each of their respective authorized representatives (collectively, “Cutera”) the absolute right to use in perpetuity and on a worldwide basis, in whole or in part, in any form or media, whether now known or developed in the future, my name, biographical information, image, voice, and any statements made by me and recorded by or provided to Cutera by me (collectively, “the Content”) for advertising and promotional purposes, or to refrain from doing so, without further notice or compensation to me. I shall have no right of approval, no claim to any compensation, and no claim arising out of the use, alteration, distortion, or illusionary effect or use in any composite form of the Content. I agree that Cutera may use, edit, copy, adapt and translate the Content as it wishes.

I hereby waive and release any and all rights, demands, losses, liabilities, claims, and causes of action whatsoever which I may now or hereafter be entitled to assert against Cutera, for myself, my heirs, executors, and administrators. For the purpose of this waiver and release, “Cutera” includes Cutera, its related companies, and each such company’s respective officers, directors, owners, employees, representatives, and agents. This waiver and release covers any damage, liability, costs, attorneys’ fees, and expenses whatsoever, arising directly or indirectly out of the recording or photographing of me or the use of my name, biographical information, image, voice, and/or statements. This waiver and release includes, without limitation, libel, slander, defamation, invasion of right of privacy/publicity or any other claim. 

By checking the “I agree to the terms above” box below, I represent that I have read the above consent, waiver of liability, and release and fully understand its contents and that I have the clear, full right and authority to grant the rights and permissions described above. I understand and agree that this Consent shall not expire and cannot be changed or terminated except in a writing signed by both Cutera and me.
